Financial Policy

To familiarize you with the financial policy of Hygiea Health & Wellness, we would like to explain to you how your service fees will be handled.

Payment arrangements:

Charges for treatment are due at the time the service is provided. We accept payment in the form of cash, check, Visa, Mastercard, American Express and Discover.

Our initial fee for a Wellness or Nutrition exam is $150

Our initial fee for a Chiropractic exam is $100

Our regular office fee is $75. Charges for treatment are due at the time of service. We accept payment in the form of cash, check, Visa, Mastercard, American Express and Discover.

Insurance Coverage

Our minimum insurance fee is $80 for regular office visits. We will submit an insurance claim form  and wait up to 120 days. You are responsible for your deductible, co-pay and services that are not covered. While we will do our best to verify your coverage, it is your responsibility to understand your insurance benefits. If your insurance company requests records, we charge $25.00 which is due prior to preparing the records.

As a preferred provider for Blue Shield, CCN and United Health, your insurance company will determine fees and reimbursement.

Pre-Pay Option

To reduce your fees, we offer a 12 visit pre-pay plan with a savings of 20%.  If you pre-pay and wish to discontinue care, for any reason, we will refund any unused monies pro-rated to our prevailing regular fee schedule.

Injuries:

If you are injured in an automobile accident we may, at our discretion, accept your case on a lien basis if you are attorney represented by someone acceptable to us. We can provide you with a list of attorneys we will work with. We reserve the right to bill med-pay and health insurance, if applicable.At this time we do not accept Worker's compensation cases.
